Quarterly Program Reports
Expense Reimbursement for Travel
Upon request, UCF Council members are entitled to receive mileage reimbursement compensation at the current State mileage reimbursement rate of $.625/per mile. To request reimbursement, follow these steps.

Complete and submit a W-9 form. A W-9 form only needs to be submitted once. NOTE: This needs to be signed in pen.

Download, scan, and email it to elise.schadler@vermont.gov or

Mail it in to Elise Schadler, FPR, 111 West Street, Essex Junction, VT 05452 or
