As of November 1st, 2020 municipalities are required by state statute to certify the appointment of the tree warden and provide contact information to the Commissioner of Forests, Parks & Recreation (24 V.S.A. § 871). This form satisfies the requirement and asks for other optional information that will allow the Vermont Urban & Community Forestry Program to best support municipal tree wardens statewide.
